We are excited to announce a new student health service available for all students at our school! Students now have access to on-demand doctor visits through our partner, Hazel Health.

All students will need a signed consent form to access this service. Parents or guardians, please complete this consent form now by clicking Sign Up at my.hazel.co/ccsd

How does it work?
Students have access to virtual doctor visits right in the school nurse's office. Hazel Health doctors can evaluate your child’s symptoms and provide the appropriate type of treatment. ● Join the ​Union Trades​ with an Adult Education Certificate
● Earn your ​OSHA 10 and 30​ certificate for free
● Pass the ​U.S. Citizenship​ exam
