Write-up: Patient is a * year old * who received a single dose of J&J COVID 19 vaccine in the right deltoid. Immediately following administration of the vaccine, patient began experiencing the sensation of heart racing, difficulty breathing, and feelings of anxiety. Patient relates this has happened with vaccines received in the past, as she has postural orthostatic tachycardic syndrome (POTS). She feels as if this is an aggravation of her POTS. Patient states she has allergies/sensitivities to ciprofloxacin, bactrim, and sulfa drugs, but she has never needed to use epinephrine. She denies chest pain/tightness, wheezing, swelling of the face/lips/mouth/tongue/throat, skin rash/itching, cramping abdominal pain, nausea, and vomiting. She does not take any medications on a daily basis. POTS aggravation. Patient was not displaying active signs and symptoms of anaphylaxis. Patient?s heart rate and breathing continued to slow down once patient was brought to a quiet place and started to enact breathing techniques. Vital signs continued to improve as patient sat. Patient encouraged to consult her PCP and inform them of their post-vaccination reaction. She was instructed to go to the emergency department if she should develop chest pain, difficulty breathing, swelling of the face/lips/airway, cramping abdominal pain with nausea and vomiting. Patient discharged to home

Write-up: I would be standing up and had to sit down because my heart started to race and was bedridden for a week (Pots Syndrome). I''m in the process of trying to get diagnosis on it to find out correlation between the two to find out if it was due to the vaccine because my health started going downhill shortly after receiving my vaccination. I have been going to a couple of Cardiologists so far and am mentioning the correlation with the vaccination to see what they are saying and it is still an ongoing investigation. My heart rate increases drastically when I stand up and goes down rapidly when I sit down within 30 seconds.

Diagnostic Lab Data: 4/16 - CBC and metabolic panel; nothing notable, slightly low sodium 4/19 - spine X-ray - unremarkable; numerous blood tests, all normal, sodium back in range 4/22 - metabolic panel, normal; spinal MRI NORMAL 4/26 - EKG; numerous blood tests, all normal 4/27 - troponin normal; d dimer 866; Covid test negative; CT for pulmonary embolism negative 4/28 - CBC, troponin, metabolic panel, EKG all normal or close to range 4/29 - MRI head, cervical spine, and thoracic spine ; all came back no adverse findings 4/30 - lumbar puncture - tested for many things , but protein came back elevated ; led to GBS diagnosis 5/23 - many blood tests - d dimer elevated again at 1546; ultrasound for DVT negative for clots 5/24 - d dimer 1693; no other symptoms of PE so Ct/ VQ scan not recommended; orthostatics tested and showed increase of 30+ bpm upon standing 5/28 - numerous blood tests ; d dimer going down but still elevated at 1496 5/31 - numerous blood tests - normal or close to for all 6/14 - numerous blood tests all normal or close to 6/17 - chest X-ray normal

Write-up: After receiving the vaccine on 3/31, experienced typical side effects noted such as sore arm, body aches, etc. - however two days after felt numbness and tingling in feet and hands which went away a few days later. On 4/14, the tingling in feet returned. Went to ER on 4/16 as tingling/numbness getting worse and progressing upward to legs; evaluated and sent home. Returned to ER on 4/22 as numbness/tingling continued to progress upward, evaluated and sent home. On 4/25, in addition to constant numbness/tingling, started experiencing tachycardia. Resting heart rate as well as heart rate doing simple tasks was high. Also began experiencing difficulty getting a full breath. Woke up 4/27 to worsening breathing and tachycardia, as well as new feelings of adrenaline rushes and Internal tremors throughout the body. PCP ordered Troponin and D Dimer and D Dimer elevated. Back to ER that day - negative Covid test - CT of chest showed no blood clots; sent home. That evening, resting heart rate was high and elevated even while lying down (90-110 bpm). The next day, 4/28, all symptoms continued and worsened as day progressed. Returned to ER and in the way experienced a pre syncope episode - upon arrival to ER blood pressure was 70/30; stabilized and sent home. Continued tachycardia and another pre-syncope episode that evening. Was referred to neurology and had appointment 4/29; all symptoms remained and were increasingly debilitating. Neurologist determined more testing needed, and was admitted to the hospital for observation/testing. Diagnosed with Guillain Barre Syndrome after head/neck/spine MRI and lumbar puncture, which showed elevated protein. Hospitalized for 5 day course of IVIG and discharged 5/4. Started feeling better for a few weeks, although numbness/tingling in legs/feet never went away. The week of 5/17 started feeling burning in throat/nose/chest and returning tachycardia; returned to ER 5/23, was admitted for observation as D Dimer high again - checked for DVT, all clear; testing and suspicion of Postural Orthostatic Tachycardia Syndrome (POTS); discharged 5/25. On evening of 5/30, experienced new concerning neurological tremors and sudden involuntary movements, as well as stiff neck/head. Returned to ER 5/31; evaluated and released. Have continued to experience autonomic nervous system dysfunction, POTS symptoms, tachycardia and bradycardia, constant numbness and tingling in extremities and occasional tingling/numbness in other areas in the body, pressure in head, internal tremors, adrenaline rushes, feelings of burning in throat/chest, suspected reflux; upcoming appointments with an Electrophysiologist and Neuromuscular Doctor for further evaluation. Note - also reported this using v-safe up until the weekly check ins were over

Write-up: On May 13th, I felt my feet go numb and I could not stand. I sat down to immediately find a heat go through my bod - heat to head. I then felt my heart racing. Took my pulse and it was 134. (I have had no heart issues at all previously - normal resting heart rate in the 60s). Three more times this happened, with heat going from feet to head (numbness in feet). Each heart rate was progressively lower with each incidence. Then it stopped. This was in the afternoon and by evening felt ok. The 14th, the same thing happened but only 3 times and with hr incidents starting at 123. Then fine again. The morning of the 15th started again, so I went to urgent care appointment in the afternoon. By this time, my heart rate was up and down.. They ran an EKG and it was fine, just showing rapid heart rate. But I was sent to the ER for further tests. At ER another EKG, chest x-ray and blood work. Only item of significance was an elevated TSH. At this point, my heart rate was going up and down (I had a finger monitor so I could watch it). I''ll note at this point, the heart rate was random but I was able to walk. Over the next week or so, hr got worse and positional. When I stood up, it would sky rocket to the 140s. I became shaky, developed brain fog, nausea, feeling like I was going to fall over and felt only well laying down or sitting. I visited my primary who initially thought anxiety but as symptoms were getting worse (I could no longer do anything: walk farther than from the living room to the kitchen, do simple tasks while standing or sitting - hr would increase while doing activities even while sitting, and no longer drive).My doctor decided it was more than anxiety when I was continuing to have symptoms that looked like POTS. I have been to the cardiologist - she did a 24 holter and EKG - no significance, but decided my heart was not the primary cause of the positional heart rate increase. I am due for an echocardiogram this week - just to make sure but she did not think that was the issue. An MRI this week, recommended by my neurologist. and have done preliminary blood work/urinalysis for possible endocrine issues - all that have come back normal so far (but seeing an endocrinologist next month anyway). I currently am seeing functional neurologist who has diagnosed me with dysautonomia (so far of unknown reasons) - he is helping me with the heart rate issue (POTS) and other symptoms that have developed over the weeks like electrical zaps throughout my body - some sort of neruopathy. I was in great health until this shot. I had not even had a cold since January of 2020. I may have hypothyroidism but it has been managed with some ups and downs in TSH for the last 20 years with no other issues. Some of my doctors do not correlate the shot to my problem but some have. Write-up: Heart rate 160+ beats per minute, chest pain, heart palpitations, low blood pressure, dizziness, shortness of breath, dehydration, migraines, hallucinations, COVID-like symptoms, chills, muscle spasms, fever of over 103 degrees Fahrenheit, vomiting, diarrhea, POTS. I have gone to the ER 5 times since the date I received the vaccine on April 6, 2021. Symptoms are ongoing. I have been referred to and seen by a cardiologist, gastroenterologist, and mental health practitioners as a result.

Write-up: first 24 hours felt run down and flu like, next numbness and tingling in hands, arm, legs, feet and face. Went to ER due to numbness and tingling on 7/24/21. Feeling worse each day. History of Postural Orthostatic Tachycardia, but has been off medicine (metroprolol) for years. Heart rate is elevated again - 150''s and even 160''s. Difficulty standing. Now like when first diagnosed with POTS when has been controlled for years. Interferring with activities of daily living, unable to go to work as must be on feet and not sitting. Even sitting, heart rate is elevated.

Diagnostic Lab Data: Date of Study: 9/24/2021 Autonomic Testing Reason for study: Question small fiber or autonomic neuropathy; postural orthostasis Heart rate, deep breathing: Normal heart rate variability for age with the average difference of 5 cycles being 19 beats per minute. Valsalva maneuver: Normal Valsalva ratio of 1.9. Normal blood pressure response to the Valsalva maneuver. Tilt study: 5 minutes of continuous blood pressure and heart rate monitoring were performed with the patient in a supine position. Thereafter, the patient was elevated to a 70? heads up tilt position for 10 minutes, then subsequently returned to a supine position for an additional 5 minutes of recording. The baseline heart rate fluctuated between 90-95 bpm, increasing to 120-125 bpm during the tilt. There was no associated change in blood pressure. Quantitative sweat test: Reduced total sweat production from the proximal leg and distal leg. Normal total sweat production from the forearm and foot sites. Clinical correlation: Abnormal autonomic studies, reflecting dysfunction of the sympathetic sudomotor fibers. Additionally, the tilt examination is abnormal, with a pattern suggestive of the postural orthostatic tachycardia syndrome. Of note, such findings can be seen within the context of a small fiber or autonomic neuropathy.
CDC Split Type:

Write-up: Initially started as feverish feeling, generalized weakness and fatigue. Went to local ED ~8 times and initially diagnosed with dehydration and hypokalemia. Came to Hospital on 7/24/21 for 2nd opinion. found to have severe orthostatic hypotension with palpitations meeting POTs criteria. seen by endocrine, neurology, cardiology and discharged after few weeks on potassium replacement and to increase hydration. Autonomic function test done in 9/2021 shows signs suggestive for POTS and autonomic dysfunction of sympathetic sudomotor fibers. Possible in small fiber or autonomic neuropathy.

Write-up: The morning following the vaccine my heart rate was elevated into the 190?s. I went to the emergency Dept where I was seen and advised to follow up with a cardiologist. I did and have been diagnosed with POTS ( Postural Orthostatic tachycardia syndrome ) I am still having an elevated heart rate despite treatment with medication. Prior to receiving the vaccine I did not have any heart issues.
